Convenzione Sistema Sanitario Nazionale (SSN) / National Health System Convention
Il laboratorio analisi cliniche “S. Anastasia” effettua gli esami in regime di convenzione con il Servizio Sanitario Nazionale (SSN) (Codice Convenzione SSN: 120-110-300701).
L’impegnativa deve avere i seguenti requisiti:
- Nome e cognome, età, codice fiscale del paziente, ASL di appartenenza.
- Eventuale diritto di esenzione dal pagamento degli esami per patologia, gravidanza, reddito o altro (a cura del medico di famiglia).
- Le prestazioni richieste per ciascuna impegnativa non possono superare il numero di otto esami.
- Sono necessari il timbro e la firma del medico richiedente.
Il Laboratorio Analisi è convenzionato con il Servizio Sanitario Nazionale e vengono applicate le tariffe previste dal Tariffario Regionale.
I prezzi degli esami sono gli stessi applicati nelle strutture sanitarie pubbliche e private del Lazio.
È possibile effettuare gli esami senza impegnativa del medico, con richiesta di prestazione in foglio bianco, ma il costo degli esami è a totale carico del paziente.
N.B.: le tariffe per prestazioni in libera professione hanno un loro tariffario e l’ufficio è disponibile per chiarimenti e dettagli.
The clinical analysis laboratory “S. Anastasia “performs tests under the agreement with the National Health Service (SSN) (SSN Convention Code: 120-110-300701).
The prescription must have the following requirements:
- Name and surname, age, tax code of the patient, ASL of belonging.
- Possible right of exemption from the payment of examinations for pathology, pregnancy, income or other (by the family doctor).
- The services required for each task can not exceed the number of eight exams.
- The stamp and signature of the requesting doctor are required.
The Analysis Laboratory has an agreement with the National Health Service and the rates set by the Regional Tariff are applied.
The prices of the exams are the same applied in public and private health structures in Lazio. It is possible to carry out the exams without demanding of the physician, with request of performance in white sheet, but the cost of the examinations is totally charged to the patient.
